Job Description

We are seeking a compassionate and dedicated Case Manager/Counselor to join our esteemed team at Bayview Recovery Center. As a Case Manager/Counselor, you will play a vital role in guiding our clients towards recovery by providing individualized support, conducting assessments, and coordinating resources.

Key Duties and Responsibilities

Client Assessments: Conduct thorough assessments to identify clients' strengths, needs, and treatment goals, and develop individualized treatment plans.

Individual Counseling: Provide one-on-one counseling sessions to address clients' addiction and mental health issues, helping them navigate the recovery process.

Group Counseling: Lead and facilitate group therapy sessions, focusing on relapse prevention, coping skills, and personal growth.

Case Management: Serve as the primary point of contact for clients, coordinating referrals, resources, and services to address their physical, emotional, and social needs.

Advocacy: Advocate for clients' rights and needs within the treatment team, as well as in external agencies and systems.

Documentation: Maintain accurate and timely client records, including progress notes, treatment plans, and discharge summaries, ensuring compliance with all relevant regulations and standards.

Client Support: Offer emotional support, crisis intervention, and guidance to clients facing acute emotional or behavioral challenges.

Education: Educate clients and their families about addiction, recovery, and resources available to support their journey.

Collaboration: Collaborate with the interdisciplinary team, including therapists, medical professionals, and support staff, to provide comprehensive and integrated care.

Community Engagement: Establish and maintain positive relationships with local organizations and resources to support clients' recovery and aftercare.

Qualification
  • Bachelor's or Master's degree in social work, counseling, psychology, or a related field.
  • Licensure or certification as a counselor or case manager in California is preferred.
  • Knowledge of addiction treatment and evidence-based therapies.
  • Excellent communication and interpersonal skills.
  • Empathetic and client-centered approach to care.
  • Strong organizational and time-management skills.
  • Ability to work effectively in a team-based environment.
  • Knowledge of relevant regulations and ethical standards.
  • Experience with electronic health records (EHR) systems is a plus.
