Job Description

Golden Gate Recovery is actively seeking a compassionate and dedicated Case Manager/Counselor to join our esteemed clinical team. In this dual role, you will provide critical support as a Case Manager, helping clients navigate their recovery journey, while also delivering high-quality counseling services.

Key Duties and Responsibilities

Case Management:

  1. Client Assessments: Conduct comprehensive client assessments to determine treatment needs, including substance abuse history, mental health, and social factors.
  2. Treatment Planning: Collaborate with the clinical team to develop individualized treatment plans, setting goals and objectives for clients' recovery.
  3. Resource Coordination: Assist clients in accessing necessary support services, including housing, employment, legal assistance, and medical care.
  4. Advocacy: Advocate for clients' needs and interests, ensuring they receive the appropriate level of care and support throughout their treatment.
  5. Progress Monitoring: Regularly assess and document client progress, adjusting treatment plans as needed to promote positive outcomes.

Counseling:

  1. Individual Counseling: Provide one-on-one counseling to clients, addressing their addiction and mental health issues, facilitating personal growth and healing.
  2. Group Therapy: Lead group therapy sessions, creating a supportive and therapeutic environment that encourages open communication and collaboration among clients.
  3. Psychoeducation: Educate clients and their families about addiction, recovery principles, and coping strategies.
  4. Crisis Intervention: Be prepared to intervene and provide support in crisis situations, ensuring client safety and well-being.
  5. Documentation: Maintain accurate and timely client records, adhering to all regulatory and ethical standards.
Qualification
  • Current license or certification as a counselor or therapist, such as Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT).
  • Minimum of 2 years of experience in case management and counseling within addiction treatment.
  • Strong understanding of evidence-based therapeutic techniques and addiction recovery principles.
  • Excellent interpersonal and communication skills.
  • Dedication to maintaining the highest ethical standards in patient care.
