Job Description

Immersive Recovery is currently seeking a compassionate and organized Discharge Coordinator to join our team. As the Discharge Coordinator, you will play a crucial role in ensuring that clients have a smooth transition from our program to the next phase of their recovery journey.

Key Duties and Responsibilities:

Discharge Planning: Collaborate with the clinical team and clients to develop individualized discharge plans that address clients' ongoing recovery needs, including referrals to aftercare programs, therapists, and support groups.

Coordination: Coordinate all aspects of the discharge process, including scheduling follow-up appointments, arranging transportation, and facilitating communication between clients and external providers.

Client Education: Provide clients with information and resources related to their continued recovery, including relapse prevention strategies, coping skills, and community support.

Documentation: Maintain accurate and up-to-date records of discharge plans, communications, and client progress, ensuring compliance with regulations and ethical standards.

Follow-up: Conduct post-discharge follow-up to assess clients' progress and address any challenges or concerns, providing support and guidance as needed.

Crisis Intervention: Be prepared to respond effectively to clients in crisis during the discharge process, coordinating appropriate care and support.

Community Engagement: Build relationships with aftercare providers, therapists, and community organizations to enhance the resources available to clients upon discharge.

Qualifications:

  • Bachelor's degree in psychology, social work, counseling, or a related field (Master's degree preferred).
  • Minimum of 3-5 years of experience in discharge planning, case management, or a related role in the healthcare or addiction treatment field.
  • Exceptional interpersonal and communication skills.
  • Compassion and empathy for individuals in early recovery.
  • Strong organizational and time management abilities.
  • Commitment to maintaining the highest ethical standards and client confidentiality.
