Golden Gate Recovery is actively seeking a compassionate and organized Discharge Coordinator to join our clinical team. As the Discharge Coordinator, you will play a crucial role in ensuring that clients successfully transition from our programs to continued care and support, setting them on the path to sustained recovery.

Key Duties and Responsibilities

Discharge Planning: Collaborate with the treatment team to assess clients' readiness for discharge and develop individualized discharge plans that include aftercare, support services, and referrals.

Client Education: Provide clients and their families with comprehensive education on the discharge process, continuing care options, and resources available for ongoing support.

Aftercare Coordination: Arrange and coordinate aftercare services, including outpatient treatment, counseling, support groups, and community resources, based on each client's needs and preferences.

Referrals: Facilitate referrals to external healthcare providers, mental health services, vocational support, and housing resources as needed.

Documentation: Maintain accurate and detailed records of discharge plans, referrals, and follow-up care to ensure continuity of care and compliance with regulatory standards.

Follow-Up: Conduct regular follow-up with discharged clients to assess their progress, address any challenges, and provide additional support as necessary.

Crisis Intervention: Be available to provide crisis intervention and support to clients who may experience difficulties during the transition period.

Team Collaboration: Collaborate closely with clinical staff, therapists, counselors, and outside agencies to coordinate and support clients' successful transition and recovery.

Qualification
  • Bachelor's degree in psychology, social work, healthcare administration, or a related field. Master's degree preferred.
  • Minimum of 3 years of experience in discharge planning, case management, or a related role within a healthcare or addiction treatment setting.
  • Exceptional interpersonal and communication skills, with a compassionate and empathetic demeanor.
  • Strong organizational and time management skills.
  • Dedication to maintaining the highest ethical standards in patient care.
