Job Description

As the Discharge Coordinator at Santa Barbara Recovery, you will play a pivotal role in planning and facilitating the successful transition of clients from our program to aftercare services and ongoing support. Your dedication to client well-being and continuity of care is essential to our mission.

Key Duties and Responsibilities

Discharge Planning: Collaborate with the clinical team to assess clients' progress and needs, developing individualized discharge plans and aftercare recommendations.

Aftercare Coordination: Coordinate referrals and appointments for clients with aftercare providers, including outpatient programs, therapists, and support groups.

Client Education: Provide clients and their families with education and resources regarding aftercare options, relapse prevention, and ongoing recovery support.

Insurance Coordination: Verify clients' insurance coverage for aftercare services, obtain authorizations, and assist with insurance-related inquiries and paperwork.

Client Follow-Up: Conduct regular follow-up with clients after discharge to assess progress, address challenges, and provide additional support as needed.

Documentation: Maintain accurate and organized records of discharge plans, referrals, and aftercare communications in electronic health records (EHR).

Resource Referral: Offer information and referrals for clients seeking additional services or resources, including housing, vocational support, and mental health services.

Crisis Intervention: Provide crisis intervention and support for clients experiencing challenges or relapse risks during the transition to aftercare.

Data Analysis: Analyze discharge data and trends to identify areas for improvement and inform decision-making.

Qualification
  • Bachelor's degree in Psychology, Social Work, Counseling, Healthcare Administration, or a related field (Master's degree is a plus).
  • Minimum of 2 years of experience in discharge planning or related roles, preferably in the addiction treatment or healthcare industry.
  • Strong knowledge of insurance verification, authorization processes, and aftercare resources.
  • Exceptional communication, interpersonal, and crisis intervention skills.
  • Empathetic and patient-centered approach to client interactions.
  • Detail-oriented with a high degree of accuracy in documentation.
  • Familiarity with electronic health record (EHR) systems is desirable.
